Pre-Surgical Agreement and Checklist

I understand anesthesia carries inherent risks to the health and well-being of my pet, and that Blue Spruce Animal Clinic will do everything they can to mitigate these risks.*

Dental Procedure

Blue Spruce Animal Clinic will make every effort to contact you before dental treatments. However, if your pet has been under anesthesia for longer than 10 minutes after contact is attempted, and you cannot be reached by phone:

I do not approve of any additional treatments and would like my pet woken up

I approve dental treatments up to $400, in addition to the signed estimate

I approve dental treatments up to $800, in addition to the signed estimate

I approve dental treatments up to $1200, in addition to the signed estimate

I approve any additional dental treatments regardless of the cost

Mass Removals

I have reviewed with the technician the specific masses I would like removed
